While generally effective, it can sometimes lead to a "cut-out," where the screw pulls out of the bone – happening in about 1% to 6% of cases. One issue is that regular screws in the DHS blade don't grip osteoporotic (weak) bone as well as locking screws. So, this study aims to see how DHS with locking side plates stacks up against DHS with traditional side plates. To do this, researchers used eight pigs (around 4 months old, both male and female, and weighing 30-40 kg each). They used Hb-DHS and standard DHS devices designed to fit the pigs' femoral neck and head.

Under anesthesia, each pig had medium neck type, Garden III type femoral neck fractures with 0.5 mm fracture gaps, and each left femur was treated with Hb-DHS and each right femur was fixed with DHS. 8 and 16 weeks after surgery, the animals were evaluated radiographically and by postmortem ocular inspection for indications of bone repair. The Hb-DHS device promotes femoral neck bone union, stimulates trabecular bone growth, raises BMD, and outperforms DHS for internal femoral neck fracture repair. This animal study will aid in the development of the best treatment for young adult femoral neck fractures.

And, because of things like the way the femoral neck is shaped, there's a chance (20-30 percent!) things could go wrong. Think slow healing, or worse, the bone not healing at all (around 15%), or even femoral head necrosis. All sorts of things can mess it uphow long it takes to get to the hospital, if someone was knocked out, whether it was open or closed reduction and internal fixation, if the internal fixation actually helps, having the right equipment, the quality of the femoral neck bone, what kind of fracture it is, how the bone slowly gets replaced, pressure building up, and less blood flowing to the area – just to name a few.

Okay, so the standard DHS? Not so great. Unlike the fancy Hb-DHS, it probably won't do much to ease pressure inside the hip or bone marrow. Plus, you can't use your own bone from your hip bone for grafting. And get thisit can lead to a bunch of problems like coxa vara, screws backing out or getting loose, and the plate just breaking! All that can be bad news for your femoral head, weakening your bones and potentially causing them to die. Now, the Hb-DHS does have its own issues, like the screws getting loose, or the lock nail and angled plate failing, plus screw cut-out and plate rupture.
